The cardholder's name, as printed on the card.

The card number as printed on the card. For Switch/Maestro cards it is the long number across the centre.

This is the type of card that you have.  
Visa Debit Cards and Visa Cards should not be confused as we ensure that the Card Number is consistent with the Card Type.

This is the expiry date as printed on the card. If your card has expired, do not use it, as it will be rejected. The format for this field is MM/YY (or MMYY)

This only applies to certain switch cards. If you are not using a switch card, do not input a value here. If you are using a Switch Card, and it does not have an issue number on it, input nothing.

If the start date is printed on your card, you must input it here. The format for this field is MM/YY (or MMYY).

This is a 3 digit number normally on the back of the card on the signature strip, normally to the left of the card or last few digits of the card number. In the case of American Express it is a 4 digit number on the front of the card normally to the right.

The address to which your card is registered, and which receives your card statements. Typically this is your home address.

The postcode portion of your registered card address.
